Tricia Schaum has been on both sides of the fence in the home health care industry. She spent many years consulting for home care businesses in the Cleveland area, and received a first-hand look from a client's perspective when her mother's advancing age necessitated employing a home heath care provider. In January 2003, Tricia founded Seasons of Care Health Services to provide local residents with the same high quality of care Tricia wanted for her own mother. Seasons of Care Health Services is a Medicare certified home care company serving the southwestern communities of Cuyahoga County. We offer a broad range of health and home support services to help people maintain well-being, independence, and quality of life. Our services include skilled nursing, home health aides, social work services, and physical, occupational, and speech therapy. Our mission is to help you live at home surrounded by the people and things you love.

Many of those we service are not Medicare patients, but just need help with the activities of daily life. They are getting older, are afraid of falling, need help getting in or out of the shower, or preparing their meals. Our home health aides are screened, bonded, and insured, and are available twenty-four hours a day to go into the home and help with bathing, dressing, grooming, meal preparation, light housekeeping, laundry, medication reminders, and errands.

Many adult children face uncertainty when a parent is returning from the hospital or a nursing home. Nobody wants to address the fact that their parents may need help. We help them collect the information they need, working closely with their parents' doctors to establish what their needs will be when they return home. We provide them with as much information as we can, and educate them on the questions to ask their parents' present health care provider as a first step in planning how to proceed.

We collaborate with each of our clients to identify their individual needs and goals. Our services are coordinated by a registered nurse under the direction of each client's physician in the comfort of their own home. If a client is hospitalized, we will come to the hospital or rehabilitation facility to assist them with their home health discharge plans. I meet each client and their family personally to develop a care plan that enables them to remain on their own, or to manage an urgent health challenge.

As a preferred home health care provider, we are proud of our connection to the local community. Our highly trained staff lives in the local area, and has developed a reputation for providing very high quality services. My pledge is to not have anybody going into a client's home that I wouldn't want taking care of my own mother.
